I don't have all the answers to your questions, but I wanted to warn you NOT to glue down the boards to the subfloor. The individual boards need to move slightly as the expansion/contraction rate of the plywood and pine boards is different. I would put down red rosin paper between the pine and the plywood before nailing or screwing the floor down.
