The main drain pipe is starting to rust, that is, there are round rust spots with noticeable rust stains running down the pipe and around the joints, where one section goes into another, there is a big build up of rust.
There is no smell as yet and the rust areas are dry not wet. What has to be done and is this a job for a professional Plumber?

If the main drain pipe as you call it is cast iron and it is less then 70 years old Get back to me in about 60 years and we can talk about what to do.
FYI many of the buildings I work in the are well over 100 years old and unless someone poured acid or corrosive liquids down the lines they should give you years of trouble free service.
The outer rust coating actually protects the piping.

I would be a lot more concerned about who installed the black ell (under the PVC TEE)
To bad the installer never went to school that day and listen to the instructor who stated.
"A clean out SHALL BE installed in ever change of direction greater then 45 deg or at the Base of ever riser /stack and any run greater then 50 ft"
It is a shame how people mutilate perfectly sound plumbing systems and devalue the property.
Clean outs do a world of good for maintaining a system.
