The most obvious code violation is that the hole for the ground prong is on the right side and everyone knows it belongs on the left. :-)'

Seriously though, the missing cover needs to be replaced, hopefully with one that will be rain tight while in use. The original cover was made for a vertical orientation and was not correctly installed. Also, it is possible that the gasket did not form a seal with the brick/mortar joint and it should be verified that this receptacle is GFI protected.
