How can I remove an entire light bulb that is "stuck"- not just part of it? Is there anything I should do to prevent this from happening in the future?

If you can't get it any other way:
make sure the power is off, wrap the bulb in something like a thick old towel or rag and break the glass. (Wear safety glasses.) Then you can get ahold of the metal base of the bulb with needle-nose pliers and twist it out.

The sockets in fixtures are aluminium and so are the light bulb bases, Purchase a tube of anti-oxidant grease for use on aluminum electrial cable (available at hardware stores) spread a very small amount on the bulb base before you install it in the socket.
