I need to change light bulbs in my foyer with a catheral ceiling. The recessed lights are about 20 feet above the floor.

Do those long reach bulb changes really work ? If yes can anyone recommend a brand/model

Yes, they do work provided that the person that installed the lamps didn't crank them in as tight as humanly possible.

McGill is one brand that I know of. You'd probably have to order on from an electrical supply house. I believe that the big box stores carry some bulb snatchers. Either way, you will need to know exactly what type of lamp you're dealing with as one size does not fit all.

How to change a light bulb in an elevated location over the stairs. Ladder is dangerous. Any ideas?
