I'd like to trim out the entrance between main hall and kitchen. By this I mean put a nice decorative molding (4"x1/4"x7') up on the edge.
Entry into room, wall is about 4" deep.

Am sure my terminology is daft.. but hope someone undersands.

How do I affix this to the wall? Is construction adhesive ok? Should I trim out the exterior wall edge too (like a window is trimmed)?

The pieces that you want to install are the casings. They are layed flat on the wall and nailed to the edge of the door frame on the thin edge with 4-6 penny finish nails and are nailed in the thick edge into studs (that are hidden inside the wall) with 8, 10 or 12 penny nails depending on how well they hold.
They need to be mitered to fit the corners of the door so that they line up on the design. They can be held nack form the edge of the frame by 1/4" if you want to add more detail to the look. Good luck.....MJH
