Toliet making noises...........

After flushing the toliet the tank fills then you can still hear the small hiss and you can see that inside the bowl water seems to be trickling in. It flushes completly and the small tube inside the tank still drips water out into the tube. Ive tried jiggling the handle, re-flushing etc. Any ideas? Thanks in advance....
